This is Medicare's page which allows you to type a CPT code to see if Medicare has limited coverage, they list the CPT code along with every diagnosis they will allow for that procedure, if the diagnosis is not on there then it won't be covered.

Aenta Provdier login in page, must have user ID & password.The Aetna website has a coding tools section that will let you see if two codes will be bundled together, this tool is really helpful, but again you have to have a login to access that tool.
